Royal Stoke probe as wrong screw put in patient

The Royal Stoke University Hospital has classified the incident as a 'never event'

An investigation has been launched after the wrong screw was put in a patient. The Royal Stoke University Hospital has launched the probe and categorised it as a 'never event'.

It follows the incident in March. The investigation is expected to be completed by mid-June.

The 'never event' was reported to the Royal Stoke's trust board this month. It related to the treatment of a bone fracture.

The report states: "Following successful internal fixation the images were reviewed as per routine and identified that one of the six screws used was a different type than required by the operator. The plan was to use cortical screws to fit the tubular plate and a cancellous screw was visible on the images.

"The key difference between a cortical and a cancellous screw is the pitch of the screw. A cortical screw has a fine pitch, whereas the cancellous screw has a coarse pitch.

"Mechanically, there is no determent to the patient's recovery and it will not compromise the fixation. There was no further impact or consequence for the patient as the type of screw incorrectly selected and used will not affect stability, fixation, pain or risk of infection."

The Royal Stoke is hoping that the incident will be downgraded following the investigation.

Deputy medical director Dr Ann-Marie Morris said: "We have strict protocols to investigate serious incidents in our hospitals and this incident is currently being investigated to identify learning and confirm action to be taken. Our staff work closely throughout the investigation period to understand why each incident occurs and how we can improve our practices."
